About identity verification documents
To prevent fraudulent contracts, identity verification of the contract holder is required. This includes document upload, IC chip authentication, and facial verification.
Identity Verification Process
After uploading your identification document, you will complete IC chip authentication and facial verification to confirm that the document belongs to you. A document review will then be conducted.
It is illegal to falsify or modify personal identification documents. If falsification or modification is confirmed, the contract cannot be concluded.
Individual

Valid driving license (front and back)
- 'Name', 'date of birth' and 'address' on the driving license and 'Contractor's name', 'Date of birth' and 'Contractor's address' must match. '
- Check if it is within the expiry date.
- When performing IC chip authentication, you will be required to enter two sets of 4-digit PIN codes that were set at the time of issuance or renewal of your driver’s license.

My number card (front side) that meets the requirements in ①.
- 'Name', 'Date of Birth', 'Address' must match. with the document
- Do not submit the back side of your My Number Card.
- Check if it is within the expiry date.

Residence card (front and back) or Special Permanent Resident Certificate (front and back) that meets the requirements of ①.
- The “Name,” “Date of Birth,” and “Address” on the residence card or special permanent resident certificate must match the “Subscriber's Name,” “Date of Birth,” and “Subscriber's Address” fields on the application.

Check if it is within the expiry date.
[If the contractor is a minor, a consent form is required]
The consent of a legal representative (person with parental authority) or guardian of a minor is required.
- Certificate of kinship (residence card or family register)
- Responsible party identity verification documents
Both (1) + (2) above must be submitted along with the consent form.
[If the policy holder is an elderly person (over 71 years old), a consent form is required]
The consent of relatives up to the second degree is required.

Both (1) + (2) above must be submitted along with the consent form.
- If the current address of the policyholder and the consenting party are the same, a certificate of relationship is not required.
- The consent of minors and relatives over 71 years old is not valid.

- Certificate of Registered Matters (Full History) or Certificate of Seal Registration
<If the contracting person is an employee of your company>
- History of all items certificate
- Certificate of Seal Registration or Power of Attorney
- Identity verification documents of the contract representative.
- The contracting person must be either the company representative or an employee.
- Please refer to the 'Individuals' section of this page for information on identification documents.
- The Certificate of Registered Matters and Certificate of Seal Registration must have been issued within the last 3 months.
- The Power of Attorney must have been created within the last 1 month.
- Depending on the result of identity verification, additional documents may be required.

About IC Chip Authentication and Facial Capture
For the IC chip authentication and facial capture of the submitted identification documents, identity verification is conducted using a system provided by Liquid Inc.
